WHAT IS REABLEMENT?
Helping you do more for yourself
Reablement is a different approach to traditional long-term care. Instead of simply doing tasks for someone, support is planned around helping the person regain skills, confidence and independence wherever this can be achieved safely.
Goals are based on what matters to you. This could include getting washed and dressed independently, preparing a meal, moving around your home more confidently, managing everyday routines or getting back into your local community.
As progress is made, the level of support can be reviewed and adjusted so that assistance reflects what you can safely manage for yourself.

Returning home after a hospital stay can feel like a significant step. Everyday activities that were previously straightforward may temporarily require more thought, confidence or support.
Reablement can help provide structure during this period by focusing on what you want to regain and supporting you to work towards those goals safely.
We can work alongside families and relevant professionals so that the support provided reflects the agreed plan, identified risks and the person’s changing abilities.

Reablement FAQs
What is the difference between reablement and home care?
Home care may provide ongoing support with everyday needs. Reablement is usually more goal-focused and aims to help a person regain or improve skills and independence over a shorter period where possible.
How long does reablement last?
The length of support depends on individual circumstances, agreed goals, progress and the arrangements under which the service has been commissioned. Progress should be reviewed rather than assuming the same level of support will continue indefinitely.
Is reablement only for people leaving hospital?
No. Hospital discharge is a common reason for reablement, but it may also be appropriate following illness, injury, a fall, reduced mobility or another change that has affected confidence or independence.
What happens if I still need care afterwards?
If longer-term support remains necessary, your ongoing needs can be reviewed and appropriate arrangements discussed with you and the relevant people involved in your care.
Will care workers do everything for me?
The aim of reablement is to provide the right amount of help while encouraging you to participate and regain independence. Staff should not unnecessarily take over activities that you can safely work towards doing yourself.
