Can care be changed if my needs change?

Yes. Care plans should reflect the person’s current circumstances and outcomes, so support can be reviewed and adjusted when needs, wishes or goals change.

Will I have the same care workers?

We aim to provide continuity and familiar care workers wherever possible, while ensuring safe staffing and reliable delivery.

Can you support with medication?
